Add Survey

  1. Open the Surveys module of the account that you plan to edit.
  2. Click Add Survey.

  3. Fill out the Add Survey page:
    • Confirm the Account. Click Select Accounts if you want to make the survey available only for certain accounts.
    • Select a Module if you want to access the survey from a module other than Surveys.
    • Type the Survey Name.
    • Type an optional Survey Description.
    • To include an end page, type an End Page Title and an End Page Description.
    • Type or select a Start Date.
    • Type or select an End Date.
    • Select a number for the Survey Quota if you want a maximum number of responses.
    • To hide the Progress Bar while a respondent answers the survey, select Hide. The Progress Bar defaults to Show so survey participants can see their progress.  This is useful for surveys containing multiple pages.
  4. Select Add Survey.
