Single Choice Question

This question, which allows for only one answer, uses radio buttons for each option.  Options are shown in a vertical list by default but different display options are also available.

Horizontal Format

This option will display the answers in a horizontal row instead of a vertical column.

Other

Selecting Other will open a text box for the respondent to type in an answer.
